CareBridge offers 24/7 access to clinical support by leveraging the trusted relationship between a patient, their family, and their caregivers. CareBridge provides each patient & their caregivers with a tablet (or mobile app) to access video visits 24/7 from CareBridge’s interdisciplinary clinical team that can assist patients with issues ranging from acute exacerbations to SDOH. Our tablet and app also have a “Red Button” that patients, their caregivers, or family members can push any time to be connected directly to our clinical team 24/7. By engaging the patient and their caregivers and empowering them with clinical resources, we have found that we can help patients stay safe and independent at home.
